Facial Fillers replace skin substances that, when lost, cause wrinkles. The skin thins by loss of collagen and fluid. In youth, the skin remains plump from a natural sugar called hyaluronic acid that attracts and binds to the body’s water. Fillers are substances that are injected to fill in wrinkles and facial depressions that may come with age. With the advent of choices for facial fillers, we can choose the best for the specific problem and longevity of effect. There are now so many fillers available, we prefer to categorize them by their ideal usage. Fillers can be used to treat three conditions:
- Superficial fine lines
- Facial folds and crevasses
- Facial volume loss
We all want fillers to last as long as possible, but don’t want bumps from over-injecting. Gradual, precise, and measured is our method. The most superficial lines, those that appear “etched” into the skin require the thinnest fillers. As the need for more volume replacement increases, fillers are placed deeper and can be thicker. Medium thickness fillers are used for actual folds in the skin and deeper crevasses, such as the naso-labial folds. Naso-labial folds run from the corner of nose to the corner of the mouth. Medium depth fillers also may be very effective for melo-labial folds that run downward from the corners of the mouth to the jaw; otherwise referred to as “marionette” lines. These thicker fillers are also longer lasting than the thin, superficial fillers.
Since there are a number of available fillers for fine and deep wrinkles, both may be treated in the same patient, and in conjunction with one another. We often layer fillers in the same areas in order to plump folds and fill the lines that may lie in the depths of those folds.
The thickest fillers are more for volume enhancement for the face that needs to be “plumped up.” As the face ages, the cheeks get hollow and less elastic. Patients may need lifting as well as volume, which re two different areas to address. In younger patients facelifts can often be delayed by filling the cheeks. This can actually give a small lift as well and is called the “Liquid Facelift.” Of course, nothing substitutes for a beautiful and professional facelift if it’s a means to elevating jowls or excess skin under the neck. But we can go a long way in making you look more refreshed by starting with fillers.
There are a number of options for facial volume restoration. The following may last for up to a year or more: Sculptra, Radiesse; Voluma, and the thinner Hyaluronic acid products, Perlane and Juvederm Ultra Plus.
Sclulptra Aesthetic, also known as Poly-L-Lactic Acid, is a synthetic sugar made up of many individual lactic acid units. Lactic acid is a substance that is naturally occurring in the body. Sculptra is a safe, synthetic, biocompatible product that restores global facial volume. Unlike traditional fillers, when injected deep into the dermis of the skin, Sculptra stimulates the body’s own collagen, creating fresh, new collagen and replacing that which has been damaged with age.
Sculptra ultimately restores the youthful contours of the face by providing a significant increase in both volume and skin thickness. By strategically restoring collagen to specific areas, we can sculpt the face by correcting hollows, wrinkles, and folds, and highlight your natural contours. Although the results are gradual, the subtle changes improve over time with repeated treatment. Most patients require a series of 2-3 treatment sessions, spaced 4-6 weeks apart. The gradual effects of Sculptra will build up over the course of each treatment, providing a natural and often undetected improvement in facial appearance.
Treatment with Sculptra is so much more than just treating a wrinkle or fold. Sculptra is used to treat all areas of volume loss such as the temples, mid-face, nasolabial folds and marionette lines. The exact areas to be treated with Sculptra vary from patient to patient, the consultation process will ensure that we address your individual needs. In general, all areas of volume loss--temples, mid-face, nasolabial folds and marionette lines--can be treated with Sculptra. Also, the wrinkling that occurs when one smiles can usually be addressed with Sculptra as it increases both the thickness and elasticity of your skin. More superficial wrinkles cannot be treated with Sculptra, it is not recommended for use under the eyes, or in the lips. Sculptra has an FDA approval of lasting up to 2 years making it one of the longest lasting options for true pan-facial rejuvenation.
What is a Sculptra Treatment like?
Sculptra comes as a powder that is mixed with sterile water for injection, this needs to be done several days prior to treatment to ensure proper hydration and dispersion of the product. We also add local anesthetic, lidocaine, to the Sculptra mix before we use it making the treatment very comfortable. After a Sculptra session, patients may have some soreness and mild discomfort that can be alleviated with Tylenol. Immediately after your treatment, you will see a sneak peak of the end result, however what you are seeing is the sterile water that we use to mix the Sculptra into an injectable form. Your body then absorbs this sterile water solution in 24-48 hours leaving behind strategically placed particles of Sculptra. You will not notice the results immediately, so it is not instant gratification like other dermal fillers. The mechanism of action of Sculptra is the production of your own collagen in response to the material, this process takes about 6 months, however, results can be seen in as little as 8 weeks. Over the course of your treatments, you will notice gradual correction and a refreshed appearance. Following each injection you will need to perform post procedure massage, the rule of thumb is 5 minutes, five times day, for five days.
Lastly, like all injections, temporary swelling and bruising may occur, to help avoid this we recommend you start taking Arnica Montana a week or two prior to injection.
Hyaluronic acid is a clear gel that already exists in the human body. It attracts water, hydrating your skin. Hyaluronic acid fillers have been safely used in Canada and Europe since 1996. Unlike many other fillers, Hyaluronic acid is non-animal-based and poses no risk of animal disease transmission or allergic reaction.
The hottest new application for Restylane is to fill dark circles under the eyes. Often, dark circles are the result of fat bulging in the lower eyelid that is exaggerated by a hollow area called the “tear trough.” By placing Restylane into this tear-trough, there is less shadow and therefore improvement in dark circles, making you look more rested and smooth.
Restylane Lyft is one of the newest dermal fillers on the market to be approved by the FDA for increasing volume in the cheeks, and the naso-labial folds. Restylane Lyft can significantly improve the appearance of the mid face by increasing cheek volume to improve contours and balance to the face. The goal is to achieve a natural appearance, with cheeks that are perfectly balanced with the temples and the jaw.
What is Restylane Lyft?
Restylane Lyft is one of the latest products offered by Galderma, the makers of Restylane. Formerly known as Perlane, this is a hyaluronic gel that is injected into the cheeks to provide volume and lift. Restylane Lyft is the only dermal filler approved to treat loss of cheek volume as well as nasolabial folds making it a pioneer in its field.
How Restylane Lyft Works
Upon injection, the gel adds volume under the skin improving hollows, resulting in immediate improvement in facial contours. Hyaluronic acid is an all-natural, skin-identical structural protein similar to collagen or elastin. It is present in youthful skin but reduces as we age. These injections supplement your natural regeneration of hyaluronic acid, which smooth out signs of aging and brings out the best of your features. When Restylane Lyft is administered into the cheeks, the hyaluronic acid molecules continuously attract moisture to the treatment area, thus helping maintain mid-face volume.
What Restylane Lyft Treats
The loss of cheek volume is a common occurrence with age, as the skin loses elasticity and the superficial fat under the skin become depleted. With reduced elasticity and less support underneath the skin, the cheeks begin to look hollow and sag. Restylane Lyft replaces the lost volume in the area to provide the skin with much-needed support, and give your face a more youthful appearance.
Restylane Lyft can also reduce the appearance of nasolabial folds, the lines that run from the sides of the nose to the sides of the mouth. These folds are the results of a lifetime of facial expressions over the years, which eventually lead to creases in the skin due to loss of elasticity. Administering Restylane Lyft will restore support to the area, reducing the appearance of nasolabial folds. Further, Restylane Lyft on other parts of the face, can add volume to improve contours including the chin and the temples.
Restylane Lyft can even be layered and combined with other filler products like Restylane and Restyalne Silk among others.
How Long does Restylane Lyft Last?
The longevity of Restylane Lyft varies from patient to patient, and will depend on the severity of the problem being corrected, as well as one’s unique biology. However, patients can expect results that last from 12-18 months.
We also use Radiesse, a non-surgical facial contouring filler that delivers an immediate, longer lasting result. Radiesse may be thought of as a longer lasting filler for deep facial folds and may even be used for volume enhancement. Radiesse appears to last even longer than hyaluronic acid fillers for treatment of naso-labial folds and marionette lines. Radiesse provides immediate visual improvement common with other fillers, along with the benefit of long-lasting results. This is because Radiesse is made of very tiny, smooth calcium hydroxylapatite (CaHA) microspheres. These particles stimulate your own collagen to grow via microspheres (particles) that form a scaffold-type platform.
Juvéderm Voluma™ XC injectable gel adds instant volume to the cheek area. Concerned with the fold that goes from the base of the nose to the corner of the mouth?
